Background:Cialis (tadalafil) is a phosphodiesterase (PDE) inhibitor. It is a first-line therapy for erectile dysfunction (ED), but also for pulmonary arterial hypertension (PAH) and diabetic retinopathy. In a large-scale trial, the drug was associated with a reduced prevalence of erectile dysfunction and sexual function (ED, 6.5% versus 9.3%) among men with ED compared to placebo (3.5% versus 3.3%). However, this is a small percentage of the total study population and does not allow definitive conclusions to be drawn. The drug has not been studied as extensively as some of its indications and the study design. We aimed to determine whether tadalafil has the same efficacy as Cialis. We also examined the relative risks of adverse events for Cialis and tadalafil compared with placebo. Methods: This is a large-scale, single-arm, randomized, double-blind, controlled trial conducted from July 2014 to September 2015 in the European and Canadian regions of the European Union (EU) and Canada. Inclusion criteria: Study population: 18,979 men aged 18 to 60 years old from the EEU and between 40 and 80 years old from the Canada. Exclusion criteria: The study was conducted in women who were unable to fulfil the inclusion criteria. Participants were randomized to tadalafil or placebo (n=15,979). Randomization was stratified by region and by treatment group. Patients received a single dose of tadalafil at 12 weeks, or a single dose of tadalafil or placebo at week 12. All-cause ED (ECDE) and erectile dysfunction (ED) were assessed using a validated questionnaire. The primary outcome was the incidence of adverse events of tadalafil or placebo. The secondary outcome measures included incidence of any adverse event of tadalafil or placebo, the percentage of patients who completed the study, number of patients with ED, the number of patients who completed the study, the number of adverse events of tadalafil or placebo, the number of adverse events of Cialis, and the proportion of patients who completed the study. The incidence of any adverse event was significantly higher in the tadalafil group (14% vs. 9%) compared with the placebo group (14% vs. 7%). Results: Patients on tadalafil had lower incidence of ED, less of ED-related adverse events, fewer patients who completed the study and number of patients who completed the study compared with patients on placebo (p=0.0023). Cialis was associated with a reduced incidence of ED, less of ED-related adverse events and a greater percentage of patients who completed the study (p=0.01). Conclusion: The tadalafil is well tolerated in patients on the EEU and Canadian market and has the same efficacy and safety profile as Cialis.
Pulmonary arterial hypertension (PAH) is a severe cardiovascular disease that is strongly associated with morbidity and mortality, especially among older adults. The incidence of PAH in the elderly population is lower than that in younger individuals, which has been attributed to various factors such as age, smoking and insulin resistance. The treatment of PAH is usually palliative, and the long-term management of this condition requires close medical supervision and close monitoring of the patient and his/her caregivers. We recently reported that tadalafil has a comparable efficacy and safety profile with Cialis. Although we did not have any information about the relative efficacy and safety of tadalafil or placebo, we suggest that this study may serve as a basis for the management of patients with PAH.The objective of this study was to assess the long-term safety of tadalafil or Cialis in patients with PAH. The safety of tadalafil or Cialis in patients with PAH was assessed in two separate studies. In one study, 10 to 20 mg of tadalafil or placebo was administered for 3 to 14 days and in the other study, 10 to 20 mg of tadalafil or placebo was administered for 2 to 14 days.
In the second study, the study was randomized to a single dose of tadalafil or placebo for 4 to 9 weeks. The study was conducted over a period of 3 years and the total sample size was approximately 50,000 in this study. A total of 6,069 patients were enrolled and had at least one dose of tadalafil or Cialis, and the incidence of adverse events was 6.5% vs. 9.3% in the tadalafil group.
